F&R Application & Verification Documents

The 2025-26 Free & Reduced Price School Meals Household Application and Verification forms are used for determining student/household eligibility for school meals, notifying households of benefits, conducting verification, sharing information, and notification of Direct Certification. 

**IMPORTANT NOTES**  
  • SFAs with year round schools should submit renewal documents and information as soon as possible in order to complete the entire agreement renewal process by the deadline, June 18, 2025.
  • Submitting the F&R Price School Meals Household Application for approval is not applicable to SFAs that have implemented the Community Eligibility Provision (CEP) district wide.
  • All SFAs operating CEP must have an approved Direct Certification letter. All families should be notified if they are approved through Direct Certification, even if Free & Reduced Applications are not collected in the SFA.

  • SFAs must submit their F&R App and Verification forms to the state agency for approval PRIOR TO distribution to households.
  • DO NOT upload the F&R Priced School Meals Household Application packet in the SN Technology System.

Prepare and submit the Notice of Direct Certification letter on SFA letterhead in the completed state as if it is ready to be distributed to households.
