UPDATED AFFIRMATION FORMS, AS OF JANUARY 2025

Before the end of January, DPI program administrators will send the new Affirmation forms directly to district federal program directors, along with details about the updates.

The 2025-2026 directory for consultations during winter/spring of 2025 is here. Directories provided on other web sites or during previous years should not be used for upcoming consultations.
