This form is for DPI staff use only. Public feedback and concerns can be submitted through the User Feedback Form.
- Web Request Form: Request additional pages, features, or navigaitonal changes. Also to request updates to the "DPInside" intranet.
Please coordinate with your division's web editor before using these forms.
General Web Request
Create, remove, publish, or edit pages. This also includes calendar event entries and correction of minor errors (broken links, grammatical errors, and typos). Please allow at least 3 business days to complete (excluding weekends and holidays).
News Release/Public Notice
News releases are coordinated through the Communication and Information Division. Public notices are legally required by state or federal entities.
Emergency notices are rare, unforeseen changes that need to be made to appropriately inform the public about an unexpected event or issue. Emergency notices must first be approved by the Section Chief of Web Services. Requests received outside normal hours will be addressed the next business day.
New Website or Website Re-design
A website redesign is any job request impacting an existing agency website including substantial changes to the site navigation, change in URL or website placement, changes to graphics, renaming or creating new navigation sections, etc. By selecting this option, customers will be contacted by the Section Chief of Web Services, to arrange a meeting in order to discuss necessary requirements.
Additions or corrections for DPInside.
Other (Forms, Maps, Data Tables, etc.)
Any other content creation or support. This includes web forms, interactive maps, data table creation, etc. Please allow at least 10 business days (not counting weekends or public holidays) for completion.
