This session will explore QIAT Indicator 6, which focuses on the integration of Assistive Technology (AT) in transition planning. Participants will examine how to ensure students’ AT needs are addressed during key transition points, including classroom changes, moves between schools or districts, shifts between grade bands (elementary to middle, middle to high), and preparation for post-secondary life. Emphasis will be placed on strategies, tools, and team-based planning practices that promote continuity and student success across all transitions.
